Does Cilk initialize N workers on the first spawn, or one worker for every spawn up until the Nth spawn? It seems better to amortize the initialization cost, but we mis-utilize resources if the spawns are sparse or few.

That would be an implementation detail, and to be honest we'd have to look at the implementation. I think both of your proposed implementations sound sane.
